If your child requires medication to be given during the school day please speak to your child's class teacher or the school office before downloading and completing the form below and returning it to school.

If your child has an ongoing medical condition that requires ongoing medication i.e. asthma, diabetes, nut allergy, etc, please speak to a member of staff as soon as you become aware of their condition. Please complete the above Administration of medication form and the Individual Healthcare Plan below and returning it to school.
